PTA Check Reimbursement Request (How-To) 

From desktop:

Volunteer > Online Reimbursement Form. 

FROM Membership Toolkit App:

You can download the app here.

1. Reimbursement form is located on the app home page.

2. Make sure to attach your receipts. You can take a photo or select from your phone's library.

3. After submitting the form, you will receive a confirmation email (similar to the parent pack receipt). Please keep the email as your record.

Registered user: Log in to the website and you can also see copies of your submissions by going to My Account > Settings > Forms
